Medical Orders for Life Sustaining Treatment (MOLST): The How and the Why

This case-based presentation will illustrate the value of completing a MOLST form as part of one’s advance directive, along with the appointment of a trusted surrogate decision-maker.   We will follow an individual through a course of declining health to demonstrate the evolution of a MOLST as a living document that needs regular review and updating to preserve autonomy through communication of goals of care.  An interactive discussion to address questions and share experiences regarding MOLST completion will follow.
