Prepare for Your Care: Advance Directives, Healthcare Proxy, and MOLSTs

Anyone, at any time, can suddenly be faced with a serious life-threatening illness and even death.  The goal of this presentation is to educate, engage and empower patients, families, and health care professionals on the importance of planning ahead for their health care, including knowing the choices they would make and sharing these choices with loved ones.  We all deserve to be treated with dignity, respect and compassion and to receive, or not receive, care that is focused on individual goals.
